The "45 mm max" seen in the top right corner of the image for top radiators is referring to the maximum motherboard component height in combination with a top mounted radiator. This mainly goes for motherboard I/O-shields, VRM heat sinks, and RAM.
The 121 mm max and 145 mm max seen for the top radiator and bottom radiator is the maximum width for radiators.
